Select the bottom one if you have existing Microsoft SQL server which is highly recommended for production.  The "New Database" means it will install a SQL express for you.

Fill in the database server name and database name. using Windows authentication is highly recommended.

Fill in the database access credential. Please take note here. If this account is changed in the future without any planning serious disaster will appear.

Test connection after entering the database credential.

Don't change anything here. The settings here are per server settings that cannot be changed in the future exception re-installation.

Don't select anything here. No option available here as this the first server.

Select "Add the Authenticated Users" and deselect "Add Anonymous users" if you don't let your application accessed by anonymous.
